SchoolCare - How to make a claim

Catholic Church Insurances Limited will act on your claim as soon as we receive your claim form. You can help us to act quickly for you by following the procedures listed below:

  • Complete all details.
  • Attach original Doctor’s Certificate. The certificate must show:
    • Name of injured child.
    • Date, nature and extent of injury.
  • In relation to Dental Claims, your dentist must provide a written statement confirming:
    • The treatment was due to an accident.
    • The extent of treatment.
    • Any future treatment.
  • Supply us with the original itemised Accounts or Receipts for claimable expenses.

Please note: Legislation in Australia prohibits us from covering:

  • the cost of any medical service for which a Medicare benefit is payable
  • the cost of any hospital treatment or other benefit unless the cost arises from an injury whilst taking part in certain activities, such as:
    • attending school
    • engaging in a sporting activity
    • secondary student undertaking a work experience program
    • voluntary services to a religious, charitable, educational or benevolent organisation
    • youth activities organised by a voluntary association such as Guides or Scouts
    • travelling to or from the above activities

Forward completed claim form to school/institution for signing. Forms must be signed by both school and parents.
